Who is Eligibile to Enroll?

All regular employees who are full-time, working 30 hours or more per week, are eligible to participate in Michael Sullivan & Associates’ employee benefits program once they have met the new hire waiting period of the first of the month following the 30 day new hire waiting period.

When Will Coverage Begin?

Upon completion of all required enrollment elections, the effective date of coverage will be the first day of the month following the 30 days of new hire waiting period.

When Must Enrollment be Complete?

New employees who are enrolling for the first time must complete all the necessary enrollment elections within the 30 days of their initial eligibility date. If the employee fails to do so, they must wait until the company’s next open enrollment period to elect coverage, unless they experience a qualifying event.

Who Can Be a Covered dependant?

These are guidelines as to who you are able to enroll as an eligible dependent. (This list is subject to restrictions and conditions outlined in the Summary Plan Description.)

  • Legal Spouse
  • Domestic Partner
  • Your Children

Domestic partners are defined as same-sex and opposite-sex couples who are registered with any state or local government domestic partner registry. Any requirements for proof of relationship or waiting periods for domestic partnerships are also applied to marriages.

Plan Year

Each Plan Year runs through August 1st through July 31st.

What is Open Enrollment?

Each year there will be an Open Enrollment period, where employees will have the opportunity to review their current benefit selections and make changes if desired. Once the desired benefits are selected, employees will not be able to make changes during the plan year unless there is a qualified change in status. If an employee is not enrolled during their initial eligibility period, they may be eligible to enroll in certain coverage's at Michael Sullivan & Associates’ next Open Enrollment period.

What If an Employee Has a Change of Status?

There are certain qualified changes in status that enable eligible employees to have a special enrollment period, if all necessary requirements are met.

  • Marriage, legal separation or divorce
  • Birth, adoption or change in custody of a dependent child
  • Death of spouse, domestic partner or dependent child
  • Involuntary loss of other qualified health coverage

Human Resources must be notified and all necessary enrollment elections completed within 30 days of the event in order to be eligible.
